Office Hours
The office is open Monday through Friday, 8:30 am - 4:30 pm. We are closed for lunch from 12:00 - 1:00.

Insurance Authorization Requirements
All patients must verify their health insurance coverage prior to their scheduled appointment.

Co-payments
Co-payments are due at appointment check-in. Payments can be made by: Cash, Check, MasterCard or Visa.
Requests for Medical Records
To obtain a copy of your medical records, a completed release must be submitted. Click Here
Request for Radiologic Studies
If you wish to obtain copies of your imaging studies, please contact the location where the studies were performed. If you have left your studies at one of our offices and would like to obtain them please call the office and arrange a time when they can be picked up.
