If you wish to make an appointment with Dr Pedlow:

You may be asked to fax or mail the following for review:
1. copies of initial and recent evaluations by other physicians,
2. copies of MRI films, CT scans,myelograms, plain films, or their written report,
3. copies of operative reports from any recent spine surgeries.

If you prefer, you may schedule an appointment with our secure on-line appointment request form. As part of this process, we would like to review a brief portion of your medical history to get to know you. Please fill out this form completely in an attempt to give us an up to date understanding of your current condition. Feel free to add any questions or other information you may have. Our office will contact you within 48 hours to review our recommendations with you.
